Currently, I do not think that there is a direct way to save Microsoft form's to PDF. While they may have a way for individual responses, it is not ideal and you may not be able to get it in the same structure. However, I feel you may benefit more if you were to create a similar form on Clappia. The process is quite straightforward and you can view this tutorial to get started:

In Clappia, once you create your form, you can also create your own customisable PDF that dynamically gets all the inputs directly from the responses. Each response will have its own PDF immediately ready to be downloaded exactly as you have customised it and if you'd like you can also integrate with Google Drive so that all responses and PDFs are available to you.

To get started in making a form:
Sign in > Click on 'New App' in your dashboard > Create the form layout in 'Design App'

For customisable PDFs:
Once the form is created, in Design App > Configurations > Print Settings
To view each PDF, go to 'Submissions' tab > Click on a submission > Click 'PDF' at the top of the right panel

You can also immediately create an app directly if you have a spreadsheet and modify it accordingly.
